Once someone is infected with SARS-CoV-2, the immune system is mobilized. As the virus replicates, cell and viral debris or virions may interact with the nAChRs blocking the action of the cholinergic anti-inflammatory pathway. If the initial immune response is not enough to combat the viral invasion at an early stage, the extensive and prolonged replication of the virus will eventually disrupt the cholinergic anti-inflammatory pathway seriously compromising its ability to control and regulate the immune response. The uncontrolled action of pro-inflammatory cytokines will result in the development of cytokine storm, with acute lung injury leading to ARDS, coagulation disturbances and multiorgan failure. Based on this hypothesis, COVID-19 appears to eventually become a disease of the nicotinic cholinergic system. Nicotine could maintain or restore the function of the cholinergic anti-inflammatory system and thus control the release and activity of pro-inflammatory cytokines. This could prevent or suppress the cytokine storm.

If we assume that in an aqueous solution all of these residues are able to react with ClO2 just like the free amino acids, then the inactivation of the viruses can be extremely rapid even in a very dilute (e.g., in a 0.1 mg/L) ClO2 solution.

Considering that human cells are not isolated but form tissues, their glutathione stock may be many orders of magnitude greater than that of a planktonic bacterium. Although chlorinedioxide in itself is a gas, it is highly soluble in water. When both air and water are present, ClO2 is distributed between the two phases in an equilibrium ratio determined by the temperature.

ClO2 gas does not necessarily penetrate viruses to inactivate them. ClO2 reacts with one or more of the cysteine, tyrosine, and tryptophan amino acid residues of the spike proteins located on the enveloped viral surface. In the case of nonenveloped viruses, ClO2 acts on the viral genome.

The H1N1 virus is effectively inactivated by chlorine dioxide.

Moreover, ClO2 is the most-used disinfectant for drinking water treatment, and its efficacy may be achieved by the routine of continuous water chlorination, as described elsewhere (0.2 mg/L of chlorine dioxide).

Similar results have been obtained for respiratory transmitted virus elimination. In particular, 0.5–1.0 mg/L of ClO2 for less than 5 min can reduce influenza viruses by 99.9% (the H1N1 virus included).
